UPIKE community preceptors are extremely important and vital to the osteopathic training of these particular students. The mission of UPIKE KY COM is to provide students with an osteopathic medical education that emphasizes primary care, encourages research, promotes lifelong scholarly activity, and produces graduates who are committed to serving the health care needs of communities in rural Kentucky and other Appalachian regions.

In order to start an academic file for you at Kentucky College of Osteopathic Medicine, we will need you to complete and return the following items:

• Adjunct Clinical Faculty Affiliation Agreement (PDF)
• W-9
• An updated copy of your curriculum vitae
• A face copy of your malpractice insurance
• A copy of your current medical license
